LifeMasters

LifeMasters provides a wide variety of disease management services for healthcare systems and health plans across the United States. Much of disease management requires a focus on an array of health related behaviors that impact patients’ health status and their ability to take care of themselves. In the majority of cases, patients are not prepared or ready to initiate some or all of the behavior changes needed. Nursing staff members are often uncomfortable attempting to intervene with patients who are not ready. The NewSof Group designed a training regimen to improve the LifeMasters’ nurses’ skills at helping patients move along the continuum of change—either on their own or through specific risk and/or disease-management interventions.
